Register your Student Organization. Every Student Organization must complete this registration form every Fall semester by September 15th.
Community Service Form. To be recognized for all community service that you organization participates in, please fill out this form.
Organizational Event Form. You must fill this out before hosting any event. You may also use the form below.
Campus Programming Proposal Form. This form is required to request funding for any type of campus event. The form must be submitted at least two weeks prior to event date.
New Officer contact Form. Each organization should submit this form anytime a change of officer position takes place and upon completion of new elections each year. It is essential to have each organization’s current officer names and contact information at all times.
Organizational Yearly Review Form. Every Student Organization must complete this yearly review form every Spring semester by April 15th.
