Academic Accomodation Forms
Once the Dean of Students and/or the Disability Services Committee reviews the Registration Form and the documentation of a student’s disability and determines the appropriate accommodations, an Academic Accommodation Form will be completed and given to the student (and the faculty member if the course is online). This form should be signed by both the student and the faculty member and returned within ten working days to the Dean of Students. No faculty member should provide special accommodations to a student unless s/he has received an Academic Accommodation Form for that specific class in that specific semester.
